Pennsylvania is preparing for new federal Medicaid work requirements scheduled to begin in January 2027, and state guidance spells out who may qualify for an exemption. The change affects some adults enrolled in Medical Assistance, Pennsylvania’s name for Medicaid.
Under the federal law, adults ages 19 to 64 who do not have a dependent child under 14 generally must document 80 hours a month of qualifying work, education, community service or related activity, unless they meet an exemption.
The Pennsylvania Department of Human Services says people who are medically frail or have certain special medical needs may be exempt. Its examples include blindness or disability, substance-use disorder, a disabling mental illness, and serious or complex health conditions that make the requirement difficult to meet.
Other exemptions can apply to people who are pregnant or recently gave birth, caregivers of young children or people with disabilities, and people who already meet certain work rules through programs such as SNAP or TANF.
The requirements do not begin for Pennsylvania recipients until 2027, but the state says it has begun notifying residents about the coming changes. People who are subject to the rule will need to report qualifying activities or an exemption as part of the eligibility process.
Federal guidance says qualifying activities can include paid work, job training, community service and education. States must also establish processes for people to report their activity and for agencies to verify eligibility.
Medical frailty is a specific legal category, not simply a description of poor health. Pennsylvania’s guidance lists the conditions and functional limitations that may qualify; residents can review the state’s instructions to determine what information their case requires.
Because the requirements are new and exemptions depend on each person’s circumstances, the state’s Medicaid changes page and FAQ are the authoritative sources for current instructions. Residents can also contact the Department of Human Services helpline if a notice or eligibility decision is unclear.
