Blue Circle Health expands free diabetes care program to Pennsylvania and beyond

Blue Circle Health, a nonprofit dedicated to improving care for people living with type 1 diabetes (T1D), has expanded its free, virtual care program to five new states — including Pennsylvania. With the addition of Massachusetts, Connecticut, Virginia, and Kentucky, the organization now operates in 16 states nationwide.

Supported by the Helmsley Charitable Trust, the six-month program offers whole-person, individualized care at no cost to participants. Each enrollee receives a personalized plan that may include endocrinology services, diabetes education, behavioral health counseling, insurance navigation, peer support, community resource connections, and financial assistance for T1D medications and supplies.

“Blue Circle Health was founded to bridge the gap between the care people with type 1 diabetes need and what is available through the traditional health system,” said CEO Leonard D’Avolio. “Our model shows that better outcomes at lower cost are possible.”

The program’s expansion into Pennsylvania comes as access to specialized diabetes care remains uneven across the U.S. Roughly 70% of American counties lack a practicing endocrinologist, and many adults face rising out-of-pocket costs and restrictive insurance coverage. Blue Circle Health’s virtual model aims to close those gaps by coordinating with existing healthcare providers and focusing on long-term management skills.

Preliminary results have been promising, showing significant reductions in diabetes-related stress after six months of participation. “Whole-person care means recognizing that true well-being extends far beyond a single office visit,” said Dr. Monica Gomberg, Blue Circle’s vice president of clinical care.

Pennsylvanians 18 and older who have type 1 diabetes and speak English or Spanish are eligible to enroll. Participants can sign up or be referred by their healthcare provider through www.bluecirclehealth.org or by calling 888-404-4813.
